They are college sweethearts and she is 5 years older than he. She was quite beautiful in college...

and apparently they are soulmates. Thus, inspite of her outer appearance he still sees something quite beautiful about her. And given the amount of adversity they've lived through together I imagine they have quite a deep emotional bond that can't be genetically acquired, surgically enhanced, or cosmetically altered thus is not replaceable by a newer, firmer, or younger model.
