Where do you live?

Once grass won't come back you have to plant, or sod. Grasses are perennials, but they do have a life span if they aren't allowed to reseed themselves.

Can't really tell from that picture but if the grass is dead in spots bugs are a definite possibility. Up here grubs are our worst enemy. Hit it with some kind of pesticide granual.

R8ers, get a dicamba type product out there. If you can get some, get an ALS inhibitor. Be careful with 2-4D and dicamba. They can volitalize and smoke your trees.
