Research Help, Part 2 -- Fewer Terms and New Format

I'm taking a class in behavioral research methods and we are developing a survey. I could use the assistance of up to 10 people. Wish I could pay you for your work, but I can't. I guess I could give out rep or my limited casino cash if anyone is interested in that.

Again, if you are willing to participate, here's what necessary.
