I agree that this is a fundamental problem. It wouldn't be as big of a problem if the mandatory coverage was more spartan. But then we would constantly be facing the problem of political pressure to increase coverages (it would appeal to politicians because it's a way to give a group something where the benefit is clear but the cost is hidden).
