Somehow "no ketchup" gets herd as "wanno ketchup" or something like that. The main thing, is that they don't understand a word you are saying, other than the keywords, such as ketchup, mayo, onions, etc.
If they hear the word, they put it in.
Next time, try ordering the burger by saying, PLAIN, with(######,#####,#####, on it)
