I politely ask people like that to either move the cigarette or put it out. Only a couple of times have I gotten the "f**k you, pal", and I will then ask the waiter to re-seat me or handle the problem.
When I go in a place that allows smoking I also try and make sure I'm not being sat on the edge of the smoking section.
