How can you have any energy to workout if all you eat is an apple and drink coffee. He lost the weight because he starved himself and actually the whole thing got pretty unsafe.
I read that after The Machinist, Bale ate and ate and gained a shit load of weight to where he became bear-like, and he had to lose weight for Batman Begins, again. Actually, they had to keep readjusting the batsuit he wore because he was constantly losing weight throughout the duration of filming.
