I think in cases where children make the type of mistakes listed here it is 99% the parents fault because there are passwords, parental controls, and other safeguards parents fail to use to limit or restrict their child's use of technology as well as monitoring their use.
Hold the kid responsible for his choices but some introspection on the parents actions is also needed.
