Poke along the area between your collar bone and shoulder blade on the shoulder that hurts. If you find a tender spot, push in and massage the area that is tender (this will probably hurt like hell). Work it for 10-15 minutes and then leave it alone for a day or two (you don't want it to bruise and swell). Repeat until the tender spot is gone or severely reduced.
There are a couple of other spots to try. One is at the base of neck on the sore side, down along the spine about three inches. The other is under the edge of your shoulder blade. You have to wrap your arm around your opposite side to pull the shoulder blade over to the outside. Then feel along the area now between the spine and shoulder blade.
I had a rotator cuff that got progressively more sore until it got to the point I couldn't lift my arm over my head. After working on a tender spot about in the middle of the upper back, between the shoulder blade and collar bone (it was almost under the collar bone), the pain almost completely went away and completely faded a couple of days later.
