The Big Play Conundrum

So we've played another game where the Chiefs have done a good job on defense only to give up a big momentum-shifting play. Statistically, the Chiefs are doing great against the run 99% of the time (if you take away the 65-yard run last night we'd have the #3 rush defense), but it got me thinking about what the cause of these long runs/pass plays actually is. Is it an overly aggressive scheme? Recurring brainfarts by players (ahem, Greg Wesley) who get out of position? And do you think we're closer on defense to the team that played in the first half against the Dolphins or the second half?
