Check/Logo help

ok so I am ordering new checks for myself and ran across the option to have "custom" checks... well, I am not yet egotistical enough to put my lovely visage on them.. so I decided to do a ChiefsPlanet theme...

Unfortunately, I am graphically retarded... do any of you guys want to help out with a jpg/gif that is at least 432 by 198 pixels that incorporates the ChiefsPlanet logo and web address?

Here's something I threw together. I can change the font, how faded it is or even the players if you want.
