Liberty Hospital lays Off 129

It's only starting. St. Luke's just embarked on $100 cost (labor) reduction plan in December. Providence in KCK whacked 100. We'll see more of it as we prepare for ACA.

So adding 16 million newly insured people to the system and hospitals are laying people off? Is that supposed to make sense?

Yes if your goal is the control of health care. Government is good.

I believe that most of these cuts are Medicare related. Medicare allowed hospitals to expand like crazy over the last 15 years or so as they ramped up for the boomers and with the cuts over the last 4-5 years they're having to trim their staff and services.

People not on Medicare were never as profitable for hospitals because they don't visit as often and insurance is getting a piece of the action.

Cutting Medicare to pay for Obamacare never made much sense but then again reducing people's cost of insurance and care wasn't addressed with Obamacare either.

I remember when they were campaigning on this and everyone was pissed about how much we are all paying for healthcare.

Its a shame about these workers, but they will quickly land on their feet. The hospital and health care sectors are booming. It was basically the one area that avoided the recession, and continues to add jobs at a fast pace. Jobs were up 28% since 2001 and are projected to increase 26% between now and 2020.

Wow. You really don't have any idea how the profit structures for hospitals operate, do you?

Medicare is generally a loss leader for hospitals due to stringent schedules set by the government. In many cases, Medicare pays less for care than private insurance does.

Hospitals absolutely hate Medicare. More and more for-profit hospitals are refusing to accept it. The only ones that are generally still taking it are state-run hospitals. Medicare is a loss-leader and numerous studies have shown that the unbalanced fee schedules put into place for Medicare are cost-drivers for those that are not medicare eligible. The chargemasters that are usually used for determining the prices for services are often inflated due to the overhead costs associated with staffing a hospital sufficient to handle Medicare volume but at drastically reduced prices dictated by the government.

No, this has absolutely nothing to do with Medicare cuts.

Physician practices have been selling out to hospitals since 1995 when the first of the large Medicare cuts went through.

Don't have the breakdown of the job classifications that were eliminated but I'll bet most were nursing supervisor type jobs. I know Liberty cut their wound clinic. That's a tough clinic to run as its just a huge money pit. Most of the folks are non compliant diabetics and they don't have the ability to pay.

But these cuts are mostly related to the Missouri Medicaid reimbursement reductions. Hospitals that have a large funding resource to lean on will hold out for a while but they'll feel the pinch soon enough too.

Yep, Medicare is determined to be the low cost leader. As their reimbursement drops per DRG, insurance companies follow suit.

Hospitals have been laying people off forever too. But just because something isn't unprecedented doesn't mean that it's not happening for a different reason now, happening at a different pace now or noteworthy in some other regard.

From The New England Journal of Medicine:

Quote:
U.S. hospitals have begun responding to the implementation of health care reform by accelerating their hiring of physicians.

...

In the 1990s, hospitals acquired many physician practices of which they subsequently divested themselves. After the current cycle of physician-practice acquisitions, it will be harder to revert to private practice if relationships sour, since new payment structures and care models will make it increasingly difficult for traditional private practices to remain profitable.
