Nothing. She's saying that if you are working and get insurance through your employer and you or a family member has a medical condition, you may feel trapped in that job because you need that insurance.

You can't, in her words, go be "an artist or a photographer" (I'm not defending those examples) or maybe "start a business and be entrepreneurial and take risk" because you can't be without or risk losing your insurance.
