The great news is that I am a dad of two beautiful little girls.

The terrible news is that those girls are in extreme medical duress.
Yesterday, my wife gave birth, via emergency c-section, at 29 weeks, two days. At sixteen weeks, what looked like a normal, but rare, identical twin pregnancy became far more complicated. Our babies were diagnosed with a disorder that happens about 15% of the time with identical twin pregnancies called Twin-to-Twin Transfusion Syndrome. A detailed medical description isn’t necessary, but a brief one may help to understand.

The placenta provides blood flow and nutrition to a developing fetus. Most of the time, identical twins draw from the placenta with little issue, but in Twin-to-Twin, there is an imbalance. One twin, called the recipient, pulls too much blood from the placenta, and also the other baby. This makes the recipient polycythemic, basically the exact opposite of anemic. With too much blood, especially red blood cells, the blood becomes sludgy, and all the extra fluid which the baby receives from the extra blood puts a great deal of strain on their system, and can lead to congestive heart failure and other problems.

The other twin, the donor, experiences problems due to a lack of blood volume.

As a result of this circulatory imbalance, there is a massive discrepancy in amniotic fluid. The recipient swims in a pool of it, while the donor is, in essence, shrink-wrapped by the lack of fluid as the amniotic sac pulls in around it.

The condition is gravely serious, and without treatment, will usually carry a mortality risk of 100%.

After finding out that what we thought was going to be a normal pregnancy was anything but at 16 weeks, we had a procedure done in St. Louis called fetoscopic laser ablation. Developed a little over a decade ago, surgeons make a small incision in the mother’s abdomen, and with a small endoscope and laser, cauterize shared vessels between the two twins. This helps to establish a normal blood flow between the two babies.
We had this procedure done on May 6.

Initially, everything looked great, as the fluid levels between the two babies balanced out, and blood flow patterns appeared within normal range. About three weeks after the procedure, our specialists noticed that the condition had actually reversed itself. About 10% of the time, the condition will spontaneously reverse itself.

It was apparent to doctors at this appointment that this had happened near the end of May. Fluid levels were imbalanced, but still at the very end of normal. As such, the doctors felt it was the best interests in prolonging the pregnancy, which was not yet at viability, to avoid another event that would require making a hole in the amniotic sac. Thus, we embarked on a pattern of extreme vigilance. At multiple appointments per week, doctors performed an ultrasound wherein they would measure amniotic fluid levels, blood flow across the umbilical cords, and within the cerebral arteries in the brain.

After this second scare, it appeared that things began to normalize. Most of our subsequent appointments looked improved, and everything appeared to be on a healthy, normal trajectory.

Until Wednesday night.

Late Wednesday, my wife began to feel a series of small contractions. While normal in the third trimester, she had enough in a short period of time that it was necessary to call the doctor. He had us come into the hospital for closer monitoring, and after doing so, noticed that one of our babies was in distress. We had three ultrasounds last night and early into Wednesday morning, and after doing so, the doctors felt that it was in the best interests of the babies to deliver them at an extremely early, 29 weeks, but still viable age.

At 5:09 and 5:10 pm yesterday, we welcomed our little girls into the world.

Because of their age, things are critical, but additional factors and deterioration in their condition have caused a number of grave concerns.
Due to what a sudden worsening of their condition, our original recipient baby, which in late May became the donor, had a seizure shortly after birth. Doctors have had an extremely hard time maintaining her blood pressure as well as the oxygenation of her blood. She has received multiple blood transfusions in an effort to both boost her pressure, oxygenation, and red blood cell count. However, due to problems associated with a hypoxic environment, she has suffered a few seizures, and to compound that further, she has been fighting the ventilator.

Her older sister is also critical, but her problems are not at the severity of those I’ve mentioned above. Since she recently became the donor, her body has far too many red blood cells, and consequently she currently has the pallor of a red grape, while her sister remains pale due to an inverse condition.

It’s hard to exaggerate the seriousness of their conditions. Don’t think of this as a pity party, as it’s not about me, it’s about our girls.

I just wanted to mention this to the entire community so that you could send out any good thoughts, vibes, or prayers that you may have, as they are in need of every one of them.

I’ll continue to provide periodic updates as the situation progresses.

We just got back from the NICU. Our less critical baby has had a fairly calm night. They are currently shining a blue light on her to help get rid of jaundice, which can be a byproduct of breaking down all the extra red blood cells she has. Our other girl has had a rough night, but she seems to have stabilized a bit over the last few hours.

When she was first born, she had a blood sugar of 1. My wife, who is a PICU nurse, told me she had never heard of a sugar this low. Most likely, that is what was causing her to seize. It's still low, but much, much better than that. For reference, I believe normal is right around 80-110.

To update you all further on the underlying condition, here are just a few more rare facts:

Identical twins happen about 3:1000 births
Twin-Twin happens in about 10-15% of identical pregnancies
After laser ablation, the procedure reverses itself (where the old donor becomes the recipient and vice-versa) about 10% of the time

The team in St. Louis who did our procedure told us that these babies were the youngest, in terms of gestational age, they'd ever worked on.

So, if you all were wondering why I've been fleeting the last few months, that's your very long and complicated answer.

Hamas my heart goes out to you and your wife. When our daughter was born she had serious complications as well and were told to expect the worst, she was a fighter and pulled through. Thoughts and prayers your way, if there is ANYTHING I can do for you guys please let me know.

Good luck and best wishes to all involved. You never really realize how much more dangerous having twins are than a normal pregnancy until you see it first hand. My friends gave birth to theirs almost two years ago. Unfortunately in the last few days of the pregnancy there was a placental abruption which caused a lack of O2 to the boy. He suffered significant neurological damage as a result and it is uncertain even now how normal a life he'll have. They are still hopeful he can compensate but the progress is slow.

As much as we give our medical system grief, we still do have the most advanced system around, they can pull wonders and I hope you get one here. Best wishes not just for healthy but no long term effects also.

I would echo this. It is not unusual for more critical intervention after 2 or 3 days. I assume they are on ventilation. Growth is a good thing. Ventilation not so much. Our thoughts are with you and your girls. I hope they allow you and your wife time to hold them as early as possible. Try to keep the area quiet with little excess noise as can be tolerated by workers.

My daughter had twin girls. She spent 6 weeks in the hospital before they took the babies prematurely. They had all kinds of issues so just be assured that the people taking care of your girls have seen it all.

My two grandaughters are 9 now. Last weekend one was doing a cartwheel that the other walked into and now she has stitches inside mouth and the outside of her chin.

I'm certain you'll be experiencing the same kind of fun twins bring someday too.

I also will pray for you and your wife. My middle son had to be hospitalized when he was a baby. It was not anything as serious as you're dealing with, but still very scary and trying. That is a very stressful time for the parents and I don't wish that upon anyone. Good luck in your situation and stay strong.

It's not going to stop us at all, but I wouldn't want to think about having two baby daughters fighting for their lives.

It's been a long, but a relatively good day. Both babies are critical, but have improved. Our more critical girl's oxygen stats have gone up, but she's still extremely fragile. Her blood sugar is much better, and she's producing a lot of urine, which indicates that her kidneys are working and starting to get rid of her excess fluid.

Our other baby is doing pretty well. She has rested throughout the day, and is acting like what the nurses say a normal 29 week old would act like, which is a celebrated improvement for us.

I know its hard....but make sure that you get some rest. When my twins were in the NICU...I wanted to be there 24/7 but the nurses made me leave and get rest. Believe me.....it helps.

My cousin had twin boys that were born early. My mom and I spent a lot of time taking care of them, I spent many a night helping out at hospital till they went home, this was while I was still in high school, she was only a year older than me. They were born early. Not as early as your girls but not much older than yours. They both go to college this year. Hope it works out for the best for you.

The first and last are our more critical girl. Her name is Hailey Grace. The middle two are of are more stable girl. Her name is Logan Nicole. She's under a blue light to help with jaundice, and the covering over her eyes is so she can rest.

If there is one thing I have learned about you in my time here...its that you are a fighter and never give up anything easily and Im sure thats a trait that your babies now possess as well.

Good luck and Im sure this is something that will end up making them very strong people as they fight through this unfortunate circumstance.

I've been around the NICU long enough to have seen some amazing recoveries, I sincerely believe that your daughters' success will be among them. Many positive thoughts being sent your family's way. My daughter spent 2 weeks in the NICU and she was so well taken care of by the team, that it put our hearts and minds at rest when we needed to physically shut down. The best of luck to your little girls, they must be fighters!

The first and last are our more critical girl. Her name is Hailey Grace. The middle two are of are more stable girl. Her name is Logan Nicole. She's under a blue light to help with jaundice, and the covering over her eyes is so she can rest.

As with all babies, they are the most beatiful girls in the world. I used to work at the Naval hospital in Portsmouth, VA, and loved spending time with the babies in the NICU. And the people who work in NICUs, like all children's hospitals, are the best people in medicine.

Hang in there man, I've spent time in those places too, and I feel your pain. Just keep pryaing and smiling for the wife. She need to see your smile, and you holding her hand.

Today has been a good day. The doctors extubated Logan about four hours ago and she has tolerated it excellently. She still has nasal prongs helping her ventilation, but she's on room air concentration. She's peeing and pooping a lot, which is also great news.

Hailey has also done well today. The doctors moved her to an oscillating ventilator since her blood pressure improved, and that has greatly improved her condition. The doctors have taken her off of dopamine, which is a medicine used to increase blood pressure, and she has been able to manage it quite well.

For those of you asking, my wife has been tremendous throughout this entire ordeal. She's handled the emotional trauma with tremendous poise, which I think has been enhanced by the fact that she's a PICU nurse.

Thanks again for all the messages of support. They make a tremendous difference.

Glad to hear. My Mom has worked in the NICU at St. Luke's for nearly 40 years. The stories of perseverance and overcoming the most gut-wrenching challenges were always amazing to me, but even more so once I became a parent myself. Best stories were of the reunions that they had, where the kids came in years later bounding around where the nurses and doctors could see them healthy and thriving.

I can also say that I am the proud father of a Daughter born at 24weeks (1lb 5oz) and spent many a days at the NICU. Your pictures look stunningly familiar. The doctors and nurses at most NICU's are the best there is. As my daughter turns 20 in 2 weeks, I can also say that while early days were tough, she is now a normal 20yr old girl.

Is this happening at the hospital where your wife works? Whether it's happening there or not, have you noticed that you're getting particularly attentive care from the nursing staff since they (I presume) know that your wife is one of them? In my experience, I've found that having a nurse with relevant knowledge in the family is a huge benefit when a close relative is in the hospital. I hope it's working to your advantage too.

Saturday, I spent 5hrs with my Niece swimming with my kids at our cabin. She was around 1lb when she was born around the same timeframe as yours.

Just wanted to give you all another update.

Milestone wise, Mom got to hold Logan for the first time today. Logan has moved to an Isolette, which is a really big improvement. She is going to transition from nasal prongs to a nasal canula today, and she's getting small amounts of milk through her feeding tube.

Hailey is also improving. She is peeing a lot, which she desperately needed to get the excess amount of fluids out of her body. Last night we changed a diaper that had eight times the pee of her first one, and she's putting out volumes far above what they were optimistically hoping without the need of potentially dangerous diuretics. She just started her first milk feeding.

They did another head ultrasound today and noticed no bleeding or evidence of bleeds in either of their brains, which is good news, but not necessarily predictive of long-term clear sailing.

Hailey should slowly transition off the oscillator today, and the doctors are slowly weaning her off of morphine, so she's waking up. I got to read a story to her this morning.

My wife should be discharged this afternoon, which they say can be quite difficult. Hopefully the transition isn't too tough on her, but she's been tremendous throughout this entire ordeal, so I'm optimistic that she'll be able to handle it.

Wow, just caught this story today. I guess I'm glad I did, given the early uncertainty and the recent good news. We had our son 5 weeks preemie, and my wife was 10 weeks preemie back in the 70's. It's rough, but sounds like you are both able to handle the experience well. Definitely have my thoughts going your way.

it has to be very stressful for a mother to be separated from her newborn. our twins were born premature, and at once were put on an ambulance and taken to another hospital w/better facilities for them. my wife didn't even get to hold them first. she was nuts.

A little over 8 years ago I posted a picture on this very site of a tiny little girl whose legs about the size of my fingers. I could easily slid my wedding band over her foot and up onto her thigh. She didn't have near the medical issues your girls have but she was about their size. She's now 8 and a picture of perfect health. I trust you'll share pictures of your girls in 8 years just as I have.

Some more good news today. They took Logan off of her ventilator yesterday, now she just has a nasal canula in, and the oxygen she is getting is room air concentration, down from 40% yesterday. We both get to hold her daily, and she tolerates it very well. It's amazing how small, but resilient she is, and even though she gets really upset when they move her, she falls asleep almost the instant we put her on our chests. It's truly a wonderful feeling.

Hailey has had a great day today. They moved her from an oscillating ventilator to a traditional vent, and the settings are lower than they've ever been. Her left lung looks much clearer, and they put in a PICC line, which means that they should be able to take out her umbilical lines in the next day or two. If things continue to progress, she should be extubated tomorrow or the next day, then we can begin to hold her.

About two days after birth she had so much swelling that her skin on her abdomen was starting to split. Apparently, this is due to the amount of blood volume she needed and the salt content of blood. Nevertheless, she's been peeing like crazy, and as she has done so, her edema has reduced greatly, and now she is starting to look a lot more like her little sister.

The steady, incremental progress has been very reassuring, even if we have a very long journey ahead of us. Thanks again for all the support.

Some more good news today. They took Logan off of her ventilator yesterday, now she just has a nasal canula in, and the oxygen she is getting is room air concentration, down from 40% yesterday. We both get to hold her daily, and she tolerates it very well. It's amazing how small, but resilient she is, and even though she gets really upset when they move her, she falls asleep almost the instant we put her on our chests. It's truly a wonderful feeling.

Hailey has had a great day today. They moved her from an oscillating ventilator to a traditional vent, and the settings are lower than they've ever been. Her left lung looks much clearer, and they put in a PICC line, which means that they should be able to take out her umbilical lines in the next day or two. If things continue to progress, she should be extubated tomorrow or the next day, then we can begin to hold her.

About two days after birth she had so much swelling that her skin on her abdomen was starting to split. Apparently, this is due to the amount of blood volume she needed and the salt content of blood. Nevertheless, she's been peeing like crazy, and as she has done so, her edema has reduced greatly, and now she is starting to look a lot more like her little sister.

It is CRAZY how resilient babies are now with modern medicine. They're like freaking Wolverine. That might be a good nickname for Logan when she gets older. ;)

Unfortunately, we are having a go-around with our insurance company because we just found out yesterday that they don't want to cover one of Hailey's treatments that they used to limit her respiratory acidosis and pulmonary hypertension because it is "experimental" for babies of her gestational age, even though the respiratory therapists, doctors, and nurses in the unit tell me that it is a standard course of care.

An unnecessary headache, no doubt.

Are they covered by your wife's insurance at her hospital? If so, does her hospital self-insure? (Or for that matter, if they're covered by your insurance does your institution self-insure?) If they do, talk to the HR people and see if they can fix this for you.

Insurance is a PIA. Keep putting pressure on them though, sometimes they can change their tune.

The good news is the girls are doing better. That's what's important.

Awesome news about the girls. As for the insurance folks, just keep fighting. They're just trying to save coin and increase their HUGE profits. As we've found with our son's surgeries, they fight for a while, then they pay. If not, they hospital will often write it off. I know Children's Mercy often leads the fight for us (they're used to it), and they DO know how to fight. Just worry about the girls, all three of them...

More good news: they extubated Hailey today, and rather than moving her to nasal prongs and staying on the vent, they moved her directly to a nasal cannula on oxygen, same as her sister. She's made tremendous progress the last week, and since she's no longer on a vent, we got to hear her cry, a very tiny one, albeit, for the first time today.
