I don't think he trusts the receivers or the OL and it is affecting his own play negatively. In some ways you can't blame him, but he's letting their failures make his play worse. That is a death spiral.

Cassel showed he is a good QB. He had receivers that can catch the ball, and on OL that protected him (somewhat).
