Would the 3 teams all be considered "co-champions"? I have an order.......

Different leagues would likely have different rules. Did one team sweep the other head to head, better divisional record, etc

Depends on the league. For instance, the Big 12 rules allow co-champion if both teams end up with the same record even if one team has the head to head win(s).
