House Lannister: Hanging on by a thread. All the heirs are dead. Jamie and Tyrion better get to it if they want the family name to live on.
House Bolton: Gone.
House Frey: Gone.
House Baratheon: Gendry is still rowing. Last of his line, I believe.
Umber: Gone.
Karstark: Gone.
The Stark family name is relying totally on Bran at this point.

Tyrell still has Willas; it should continue. He's been used as potential marriage bait a time or two; I expect he'll be married off still (Dany, perhaps?). EDIT: Tyrell also has Garlan Tyrell. Neither have been part of the show but are in the family tree issued by HBO so part of the show canon nonetheless. The Tyrell line is still in fine shape (recall that Loras was Kingsguard in the books anyway and was the youngest brother; he was never going to be the lord of Highgarden). Actually, it looks like the current HBO website shows Loras as the only son. That wasn't previously the case. So book and show have diverged quite a bit here. The Tyrell line is all but extinct on the show (very very odd and an unnecessary mistake, IMO).
Frey also still has some remaining males, or so I believed. Umber and Karstark may/may not be wiped out, we can't be entirely sure. They surely won't continue to exist as cadet houses, but I don't know that their lines are truly wiped out (I could actually see the Karstark's mending fences).
Bolton and Baratheon are done. Gendry is still bastard born and I don't expect Daenerys will be eager to legitimize him and bring back the line that she blames for the murder of pretty much her entire family.
