Every year GMs fail. Even very experienced GMs fail, like Pioli. It is a harder job than you make it out to be. Matt Millen was everything John Lynch is - spent his life playing football, then took a TV job, no FO experience. He is very possibly the worst GM in football history.
Name a GM with no scouting, coaching or FO experience that turned out good.
