No. This isn't Tuskegee. You inoculate enough people to reach a threshold of certainty that numerous people would be infected. From there, you are able to ascertain if infection occurred and how serious it was.
When testing Truvada for PrEP they didn't tell men to have unprotected sex with guys with HIV, but the groups were likely to have that occur enough to document a certain level of infection. When those were nearly nonexistent, they were able to say with conviction that it is effective for PrEP.
