I think part of the reason for that is because Italy has one of the most eldered populations in the world.
This virus tends to feast off eldered people with weakened or impaired immune systems and/or people that suffer from several comorbidities.
But in no way do I believe Italy has more deaths than China. It wouldn't surprise me if Chinas death toll is in the millions.
