I don't even think the policy at Rain Man, Inc. (not there these days, but I managed some of that stuff) allows for individual employees to opt out. It's an all or nothing thing. As RM alluded to earlier, it's like $5/month/employee for both STD and LTD, so it's pretty easy to throw it in.
According to SHRM, 78% of employers offer it, though they don't clarify if some employers somehow make employees pay for it.
