Well, I can speak professionally on this, but with the caveat of course that everyone is different and I dont know your specific patient info. so here is the best I can do. The blue you see from the tooth could be from a silver filling in the tooth showing through. If you see one in the top of the tooth it is very possible. Ask him to prescribe 10 mg valium for your appt. It is antianxiolytic and will turn the phobia down a few notches. Dont let him put anything silver colored in the teeth as far as restorative materials go. If it has been 6 years, you more than likely need periodontal treatment rather than a standard cleaning. If the dentist does the cleaning, and not a dental hygienist, find a different office. I never schedule patients with a cleaning the same day as a new patient consult for that very reason. If a cavity is larger than about 1/3 the width of the tooth, it should have a crown. They typically run anywhere from $750 to $1000 and insurance may pay up to 60%. Hope that helps.
