The two most glaring things are Pioli's insistence on keeping Cassel (and then building the rest of the team around his weaknesses -- better RBs more protection, tall circus-catch receivers) and putting Romeo at HC and then NOT even hiring a DC.
Two "let's keep our fingers crossed and hope this works" moves. Not "win at any cost" moves. And teams who win make changes to win. It just feels like a "do the least we can" mentality, and that's what's the most frustrating for me.
