In my world no one should lose all their vacation time for a protracted illness. Not the person who got sick, or anyone else. PTO should be for short term illness only.
Oh you got bone cancer - well say goodbye to that month of vacation time you saved up for 2 years, and start begging for PTO donations.
There are better ways to handle it, as has been discussed.
