Probably not the grossest but the first one I remember that gave me the chills was in Die Hard when Bruce Willis jabbed that icecicle into that dudes eye! Didn't see that one coming.
Mel Gibson's hatchet job in "The Patriot" was pretty nasty too. Most "gross out" scenes today are too over the top to be effective IMO.

I can't narrow down a specific scene, but I can say that Visitor Q is the sickest movie I have ever seen.

Granted the following has a Warren Zevon song over the clip so you can't hear the metal on bone from the actual scene but nonetheless I give you fair warning that the following scene from IRREVERSIBLE is still quite sickening.

Just my .02 but I saw Dead Alive as comedy/horror if you see Irreversible the mood during that scene is gut wrenching,there's also a rape scene in the movie that is the most realistic thing I've seen on film,shows the sheer brutality of the crime.
