It's literally the last shot before the commercial at the 35 minute mark.

She says "We'd be doing him a favor," drops the Stevia in, then the camera changes to a shot directly above the tea and zooms in on the tea as the Stevia disappears, along with dramatic "whooshing" noises.

I just read the post where it said it was done off camera. But in that scene Walt is never off camera. So they're implying that he slipped it in there at some point by her stirring in the sugar packet, but it wasn't ever shown "how" he managed to slip it to her..at least I think that's kinda important if you're just going to come out and say later that "I slipped you the ricin that's why you feel very ill." IMO that was poorly done unless I'm just completely missing something and need to watch again on dvr.

Knowing that she's a creature of habit he can easily make a miniscule hole in the upper portion of the packet to pour a a few grains in. He could have adulterated Stevia packets before her meeting and placed the killer one in the container before she went to the diner. Notice how there was only one packet there?

Not everything has to happen on the screen.

Did Walt teleport to the coffee shop or did he drive there off camera? There are reasonable assumptions viewers can make about what did and didn't happen, especially when given visual and spoken word evidence about what happened.

Walt can still be despicable and right wrongs, and that's what this episode attempted. He had motivation and rationale to do it.
I think most people view Walt's death as redemptive. Which goes to the "lack of depth of his loss." I thought Gilligan wrapped up his storyline way too conveniently.

He knows he's ruined lives. But in the grand scheme of things, he got off incredibly light. Hank was killed off without getting closure while Walt had a full episode to tie up loose ends. Jesse was distanced from his family, lost a best friend, and had 2 girls he cared about killed (one... COMPLETELY innocent). Walt didn't lose anybody... just a brother-in-law he didn't care about protecting until the last minute.

Walt was allowed to have an epiphany and fix everything wrong without any majorly devastating event to trigger it. He got off light. I think it would have been a much more complex ending if he had to die knowing that one of his loose ends that really mattered to him weren't tied up.
