Embedding forms in Outlook message?

I am trying to embed a customer quote inside of an outlook message.

I would like to save this and use it company wide for employees to be able to open this created form email, fill in the necessary info, send it to customer, and have the customer hit an accept or decline which would generate a response back to employee here.

Anyone know if this is possible and if so, where the best source to learn on it would be? I am google-ing, but not having much luck.

you/they could use voting buttons; 'approve' / 'decline' that sort of thing.

This particular article is based on conducting 'polls', but you get the idea/concept etc.

I don't think what you're wanting to do is possible without involving additional software besides Outlook. Especially if you have no control over the customer's e-mail systems.

And like Saulbadguy said, depending on your e-mail server, and your user's e-mail server, putting that kind of behavior in an e-mail is a quick way to get marked and blocked as a spammer.

Yeah, it's probably possible by creating an outlook template and sending it to everyone (or pushing through administrative templates)...but I don't recommend it, it seems like you are trying to use e-mail to do too much.

It seems like it would be easier to just set up a simple website with that stuff.

If you feel so inclined, Access can create a template and when replied to can automatically add the information to the database.

Yes, the more I research, the more I find that the things I am wanting to do would only be (possibly) possible in house.

Next step I guess is to start researching salesforce.com and other such websites or setting up a simple one as Saul suggested.

Learn some VB. Depending on your version of Outlook, Forms Scripting is pretty robust.
