They had a whole episode of some crime/news show on this, and as I recall, they had a gun with the blanks, but they also needed a close-up shot down the barrel where you could see the bullet in the chamber. So they had this 'blank gun' with the tip of a bullet in front of the blank and Lee didn't know the bullet tip had been put in there for the close-up.

They loaded the pistol with dummy shells that had the powder removed from the casing but still had real bullets at the end. One of those bullets dislodged from the empty casing and got stuck in the barrel of the pistol. The weapons handler (who is the one who bears true responsibility for this incident) didn't notice that the bullet was missing when he removed the dummy shells. After that, the same gun was used with blanks, which have no bullets but powder in the casing. When the trigger was pulled, the powder in the blank propelled the dislodged bullet forward just like a normal shot.

Very sad.

The guy who played Fun Boy was the one who pulled the trigger. Even though he was absolutely not to blame, that sure must be tough to live with.

Brandon Lee died during a mishap on the set. A scene required a gun to be loaded, cocked, and then pointed at the camera. Because of the close-range of the shot, the bullets loaded had real brass caps, but no powder. After the cut, the props master (not the arms master - he had left the set for the day) dry-fired the gun to get the cock off, knocking an empty cartridge into the barrel of the gun. The next scene to be filmed involving that gun was the rape of Shelly. The gun was loaded with blanks (which usually contain double or triple the powder of a normal bullet to make a loud noise). Lee entered the set carrying a bag of groceries containing an explosive blood pack. The script called for Funboy (Michael Massee) to shoot Eric Draven (Lee) as he entered the room, triggering the blood pack. The cartridge that was stuck in the barrel was blasted at Lee through the bag he was carrying, killing him. The footage of his death was destroyed without being developed. Lee is the son of martial arts legend Bruce Lee, who died in mysterious circumstances before completing Game of Death (1978). See also Dragon: The Bruce Lee Story (1993).

I had just watched the movie "Lords of Dogtown" in which he played a great role.
