I have worked on various shoots as a grip, and a PA. As a grip, I was told by the gaffer what light to set up and where. Although most of these weren't movie shoots, they weren't low-budget, either. We never had a best boy. I was told long ago that a grip creates light, and a gaffer perfects it.
The gig that I loved that wasn't in the poll was that of being a dolly grip, which is the guy that pushed and pulled the dolly with the camera mounted on it on the tracks.
