Didn't see this posted anywhere yet, but wouldn't the holder be down when he picked up the ball?

depends if it crosses the Line of scrimmage. I have seen plays where the ball is kicked and it is blocked behind the line of scrimmage right back to the holder who is still on his knee. he then holds it as the kicker backs up and kicks it again.
