Wasn't there a pitcher for or against WSU where he was warming up, noticed the guy in the on deck circle counting his pitch speed, and threw a fastball at him hitting the side of his head?

You're referring to the when Ben Christensen beaned Anthony Molina, who was only 24 feet from home plate as he was warming up, while the on deck circle would typically be about 37 feet from home plate, although batters usually are just in the vicinity of it (https://www.nytimes.com/2010/10/05/s.../05ondeck.html) .

Molina was due to lead off an inning for the University of Evansville. As Christensen warmed up on the mound, Molina took practice swings 24 feet from home plate. Wichita State's coach, Gene Stephenson, taught his pitchers to "brush back" on-deck hitters standing too close. Christensen threw a pitch at Molina's head, the unsuspecting victim looked up just as the ball collided with his face. Molina was taken to the hospital and treated for his injuries. The hitter's vision fell from 20/10 to 20/400. Christensen was suspended for the remainder of the season.
In 2014, Molina was sentenced to a 6-year term in prison on the basis of an Alford plea (https://en.wikipedia.org/wiki/Alford_plea) to 2 felony counts of sexually assaulting a 8 year-old girl.
