Closest teams that I could find fitting that criteria since 2000 would be the 2000 Ravens, 2001 Patriots, 2002 Buccaneers, 2003 Panthers, and 2012 49ers.

Billick was in Year 2 as HC, then Dilfer started, who was not on the team the previous season.

2001 Patriots was Year 2 under Belichick and the legend of Brady started after Bledsoe went down.

2002 Buccaneers were in Year 1 under Gruden, but Johnson had played the year before in Tampa but didn't start all 16 games.

2003 Panthers was Year 2 under Fox and Delhomme was in his first year as the starter there after being signed in free agency.

2012 49ers were in Year 2 under Harbaugh and Kaepernick took over for Smith after he was concussed, but Kaepernick was in Year 2 in the system.
