It's been awhile since I had an eye exam, but trying to remember. They lower that thing to your face and if I'm remembering the person conducting the exam is up close and flipping lenses back and forth asking "better or worse" as the person being examined answers in repeated fashion whether the image is more or less blurry. It's fairly up close and personal. If he seemed nervous and shaking his leg early on she might not have caught that he had moved on to beating off. Asking if it was vomit might have just been a moment of complete confusion when she noticed something on her leg. Given this guy's history I'm inclined to believe whatever story she told.
